By submitting this form, you agree to cooperate with BizStarts to the extent possible in responding to surveys and permitting the use of such data, photos of you or your business, and social media photos and comments in BizStarts reports and publications designed for educational or promotional purposes.
Furthermore, nothing conveyed by BizStarts, its staff, consultants, mentors, volunteers, and board members (collectively, the "BizStarts Team") shall be construed as legal or tax advice. Any business advice provided by the BizStarts Team should be evaluated by the entrepreneur, the business's owners and management team within the context of the professional advice you receive from your accountant(s), attorney(s), board of directors, and other professional adviser(s). By submitting this form, you, both personally and on behalf of your business/company, heirs, successors or assigns, agree to release and hold the BizStarts Team harmless from any liability or claim made as a result of said business advice, actual or claimed errors or acts of the BizStarts Team, including but not limited to claims of negligence, or any omission thereof.
